If you are younger than 40, you are not eligible to participate
Screening will be provided only to 150 pre-registered participants who have successfully completed their electronic application ONLINE.
Registration will close as soon as the 150 spots are filled or, at the latest, by midnight on Monday, May 12, 2025.
No Paper Application will be accepted.
Please fill out one application per person and pay a $10.00 per application refundable deposit online as per the instructions. Please note that the $10.00 deposit will be refunded only if the applicant is present on the day of the camp and expressly requests it on the day of camp.

RWJUH Hamilton will provide blood pressure check, diabetes screening via Hemoglobin A1c, balance screening and informational materials about the hospital’s outpatient services at the health fair.
Notes:
- 12 hours of fasting is required for blood tests. Please see the details in your registration confirmation email. Continue all medications the day before and the day of the blood test. Bring all your medications with you to the health camp.
- Breakfast will be provided after the blood test is completed.
